1. Philippine Passport

A broken passport is not accepted. Passports must be signed and must have at least two (2) blank visa pages.

3. Photo

Specs: 4.5cm x 3.5cm, with white background.

Photo must be taken within 6 months.

Please write the applicant’s name and birthdate on the back side of the photo.

The photo must be pasted on the application form.

If there is a used Japanese visa on your passport, Birth Certificate and Marriage Certificate are unnecessary.

4. Birth Certificate

IT must be issued within one year from PSA Main Office/Serbilis Outlet Center (Nationwide).

If the birth certificate from Philippine Statistics Office (PSA) is unreadable or has incomplete Information

Please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.

If the birth certificate is “late registration”

Please submit in addition, a baptismal certificate and school record (Form 137) from high school or elementary and School Yearbook (if possible).

If there is no record in PSA

Please submit a “Certificate of Non-Record” from PSA together with a birth certificate from the Local Civil Registrar.

5. Marriage Certificate (if the applicant is married)

IT must be issued within one year from PSA Main Office/Serbilis Outlet Center (Nationwide).

In case there is no record in PSA

Please submit a "Certificate of Non-Record" from PSA together with the marriage certificate from the Local Civil Registrar.

8. Personal Bank Certificate and 6 months bank statement

[if the applicant will shoulder part or all of his/her travel expenses]

The validity of the Bank Certificate is three (3) months from the date of issue

9. Photocopy of Income Tax Return Certificate (BIR Form) - It must have amount of gross income and a stamp of the recipient, 

- For person subject to income tax withholding, the Certificate of Compensation Payment/Tax Withheld (BIR Form No. 2316) must be signed by the employer and employee. 

- For tax payers other than above, the Annual Income Tax Return (BIR Form No. 1701, etc.)  must have receipt stamp of the tax collector.  

- If tax is not withheld (e.g. self-employed, housewife, retired) and if the Average Daily Balance (ADB) is not stated in the Bank Certificate. Submit Bank Statement to prove transactions for the last 6 months 

10. The Certificate of Employment should have the following information (Date hired, Position, Compensation and name/contact information of the certifier), otherwise the applicant has to provide an explanation letter.

11. Guarantee Letter (original)

[If Guarantor who lives in the Philippines or outside the country will shoulder part or all of the applicant's travel expenses (either Filipino or Foreign passport holder]

12. Proof of relationship between applicant and guarantor is/are required even if the applicant has a previous Japan visa.

(e.g. Birth Certificates, Marriage Certificate, Documents like pictures together, Certificate of Employment, etc)

13. Guarantor's Personal Bank Certificate and 3 months Bank Statement - original (if the account is in the Philippines) or Statement of Account (if overseas account)

14. Guarantor's Income Tax Return (ITR Form 2316 or Form 1701) (clear Photocopy)

15. For Housewives, Retiree, and Senior Citizens who will provide their own financial documents, they need to submit an explanation letter regarding their economic activities to justify their financial capacity.

16. For Incentive trip sponsored by a company, the Bank Certificate should also have the 6 months Average Daily Balance in Bank Certificate, otherwise 6 months Bank Statement has to be submitted.
